Proof of Residency Required:
All students must bring at least two of the following items that show the parent or guardian's name and address:
- Sale contract
- Real Estate Tax receipt
- Mortgage statement
- Current MLG&W bill
- Lease/Rental contract
- Military letter of assignment
Additional Documentation:
- Previous year's report card (not applicable to students entering Kindergarten)
- Social Security Card (requested)
Kindergarten Requirements:
Kindergarten students and new students must bring the following additional documentation:
- Certified copy of birth certificate
- Proof of medical exam within the last 6 months
- Tennessee Child Health Record or Temporary Certificate of Immunization
- Child's Social Security number
Proof of Immunization (New Enrollees):
Children who are new enrollees in a TN school in grades other than Kindergarten or 7th:
- Diphtheria-Tetanus-Pertussis (DTaP, or DT if appropriate)
- Measles, Mumps, Rubella (2 doses of each, usually given together as MMR)
- Poliomyelitis (IPV or OPV) – final dose on or after the 4th birthday now required
- Varicella (2 doses or history of disease) – previously only one dose was required
- Hepatitis B (HBV) – previously only for Kindergarten, 7th grade entry
Proof of Immunization (Kindergarten):

- Tetanus-diphtheria-pertussis booster (“Tdap”) – not required if a Td booster dose given less than 5 years before 7th grade entry is recorded on the DTaP/Td line (no later than October 1, 2010)
- Verification of immunity to varicella – 2 doses or history of disease (July 1, 2010)
