Physicals and Immunizations

In Tennessee, children are required to have certain immunizations before entering specific grades. In addition, a child who is new to Tennessee schools must have proof of a recent medical examination. The Memphis and Shelby County Health Department can provide these services. If your child is covered under Tenn Care, the services are free. Children not covered pay $85* for a physical exam which include any immunizations given at the time of the exam. Immunizations alone cost up to $35* per visit.

The following immunizations are required:

  • Immunizations Required for All Students (K-12) - 2 rounds of MMR, both to be received after the child's first birthday; 4 rounds of DTP, one of which to be received after the child's fourth birthday; 4 rounds of polio.
  • Additional Immunizations for Entering Kindergarten - Hepatitis B and Chickenpox
  • Additional Immunizations for 7th Grade - Hepatitis B (if child did not receive as a baby)
